South Windsor Transition Program (SWTP)

Program Overview

 

The SWTP is South Windsor’s public transition program and is currently located in the South Windsor High School Annex. SWTP serves a small cohort of young adult learners, aged 18-22, who are determined through the PPT process to need additional support in the areas of post-secondary planning, vocational training, and life skills development. Our program philosophy focuses on individualized learning, community engagement and person-centered planning.
